There is chronic pain of whatever kind, and there is acute pain. Chronic pain, one can learn to deal with, to rise above it as the alternative to sinking under its weight. I imagine that's what he's talking about. But agony, acute pain, it is hard if well nigh impossible to think of anything else until that suffering is relieved.
